As a e-representative of myparklane.com, a subsidiary of JBPL, I understand and agree to the following terms: 
          1. Upon acceptance of this Agreement by JBPL, I will have the right to sell JBPL product in accordance with the JBPL compensation plan. I have read and agree to abide by the JBPL compensation plan document, which is incorporated as part of this Agreement. I understand that the JBPL compensation plan document may be amended from time to time and I agree to be bound by all amendments when published in official JBPL literature.
              2. I acknowledge that I am (a) an independent contractor and not an employee of JBPL for Federal tax purposes;
                (b) engaging in an independent business and not covered by JBPL under any federal or state unemployment or workers compensation act or for social security benefits; (c) responsible for payment of all federal, state and local income taxes arising out of my activities and for filing of all returns and reports; and (d) responsible for compliance with state workers compensation laws. You must notify websupport@myparklane.com before or within ten days of the Renewal Date on your contract if you wish to cancel an account. Park Lane or eNetwork-Solutions does not offer prorated refunds for cancellation requests submitted after ten days of the renewal date. Any questions email websupport@myparklane.com *all site are subject to a $50.00 set up fee unless otherwise stated. Refer to myparklane.com manual. **  The Jewels by Park Lane "myparklane.com" program is a one year subscription. All payments are to be paid in full and any delinquent payments and subject to a service charge. All sites are renewed annually and may only be canceled at the anniversary date. By submiting this form you have agreed to these terms of the myparklane.com program. All charges will appear as Jewels by Park Lane Inc.
